The title of Golden Goddess was designed for the accomplished entertainer who reached the age of 50 years or better, regardless of how many other titles she may have accumulated. Contestants must compete in 3 divisions: Blue Presentation, Talent, and Gold Gown(in keeping with the theme of Golden Goddess). I understand that I will not be an official contestant of The Hawaii Boa Arts Ball Golden Goddess division until this form, entry fee, headshot, and program ad are received by June 4, 2019(by 11:59 pm HAWAII TIME). I am aware that there will be a 10 (ten) point deduction should this form or entry fee be turned in late. I also understand that if I do not make the scheduled final drop off below, it becomes my responsibility to get these items to the producers.
